Which documents are required for taking an insurance plan?

The following documents are required for all insured members: 

• Proof of age
• Proof of identity
• Proof of address
• Proof of the policyholder's income
• Duly filled proposal form

Some health insurance plans may require the insured members to undergo Medical Tests, and the report of the same will be required too.
